This program is specifically designed to evaluate whether client should remain home or in an alternative housing.

While we can say that natural phenomena can be, in many cases, shocking and unexpected, we at Scoop for Seniors can help families and patients navigate the healthcare system by becoming their healthcare   liaison and advocate.  In many cases, just trying to acquire the correct or accurate answer is what a family or a patient may need along with the ability to speak to a professional while receiving the correct information concerning the benefits provided by your healthcare insurance.

$375 (4 hours)

  1. Initial consultation (usually over the phone) with client and/or family (30 min)
  2. On-site bio-psychosocial assessment/evaluation of client which includes physical, mental, emotional and social status and needs (90 min)
  3. Gathering of additional information, if needed (30 min)
  4. Development of personalized report/plan of care (1 hr)
    • includes summary of bio-psychosocial assessment
    • list of all the findings, presenting problems and issues
    • evaluator’s recommendations
    • detailed step by step plan of care
    • available benefits
    • appropriate recommendation and referrals to services for home care or professional services and all other pertinent resources information.
    • Delivery and presentation of report to client and/or family (30 min)

    (Delivery of report is usually done by e-mail or mail and presentation is done over the phone or by Skype.)

    TRANSITIONAL SERVICES COORDINATION

       

    Transitions can be difficult and complex, especially when it involves healthcare issues, insurances, crisis and diverse interventions.
    Scoop for Seniors specializes in diverse types of transitions that a person may find themselves in during the course of their lives.
    A transition can consist of transferring a patient from a local nursing home to another local skilled nursing facility, or, in some cases, from one state to another. Some transitions can involve alternative housing such as Assisted Living Facilities, Independent Living Facilities or Metal Heath Care Units.  Scoop for Seniors specializes in the transition of clients from out of state to Southeast Florida, coordinating all aspects of services and benefits.
    At times, families would like to move their loved ones out of Florida and closer to them.  Without the correct coordination of services and benefits, this can become a nightmare.
    Scoop for Seniors, through this new Transitional Program, helps families achieve their goals with a holistic and organic approach.
    Out of state transitional coordination is our specialty. We can help you coordinate all the services that a person may need while coordinating all the benefits needed to have a smooth and successful transfer.
    Our Coordinator will evaluate your situation and assess the services available to you and your loved one in Southeast Florida.
    Understanding, for example, the Medicaid and Medicare regulations and guidelines is part of our expertise.  We are able to help families with their needs, preventing unexpected financial problems and prevent loss of benefits.
    At Scoop for Seniors, we have witnessed many cases where clients or patients were not able to achieve their highest level of functioning due to the lack of coordination of appropriate services.
    A transition can also be more than just a physical move. Some transitions can involve a new disability or a new behavior. This can be overwhelming and very difficult to adjust to, not only for the patient, but the family as well.  
    With our Healthcare Network System and Coordination of Services, Scoop for Seniors can also assist a client who wants to remain in their home. Sometimes, this can be the safer and healthier way to go. We will make sure the person at home has appropriate services and support.
