This program is designed to help families navigate the complicated and sometimes overwhelming healthcare system. The assigned coordinator becomes the liaison, the spokesperson, the facilitator between the family and the healthcare professionals involved in the care of the client. Scoop for Seniors specializes in understanding changes, new needs and new desires while creating solutions for patients, caregivers, families and healthcare professionals. We believe that all situations should be approached with a holistic and organic understanding.  At Transition Benefits and Services Solutions, we view a crisis, a problem or a new intervention as a WHOLE.  We believe in looking at the whole picture, not just a portion of it.

   The Healthcare Family Liaison program was designed to help families navigate the complicated and sometimes overwhelming healthcare system. The objective of a Professional Coordinator becoming the liaison, the spokesperson and the facilitator between the family and the healthcare professionals is to achieve fast, effective and crucial information for the family.  The goal is to manage the entire situation and gather all of the information that is difficult for a patient to receive on their own.   Knowing the healthcare system and how the professionals in the industry operate under diverse regulations is what sets us apart from others. This service proves over and over how families save money and clients improve their quality of life due to the excellent and correct communication between the Professional Coordinator, the healthcare professionals, the client and their families.

Care management services are offered in a variety of settings and we concentrate in Short-term or ongoing assistance for long-term caregivers. We personalized services designed specifically to meet the client's wants and needs. The continuity of care management to reduce miscommunications, time, stress, and costs to clients it is the program goal.  We believe in efficiency and flexibility because care management services are streamlined and client-centered, not complicated by bureaucratic red tape.

$375 (4 hours)

Initial consultation (usually over the phone) with client and/or family (30 min)

  1. Situation, event or intervention evaluation which includes review of medical records, if available, interview with client and gathering of accurate contact information from all healthcare professionals involved in the client’s specific situation (1 hour)
  2. Initial intervention/contact with healthcare professionals, such as physicians, social workers, administrators, nurses, home health care services, or rehabilitation services;  may require written authorization from client, power of attorney or health care surrogate (90 min)
  3. Preparation of written report based on the situation evaluation and the results of the initial intervention with the healthcare professionals (30 min)
  4. Delivery and presentation of report to family (30 min); Delivery of report is usually done by e-mail or mail and presentation is done over the phone or Skype.

OPTIONAL: Additional intervention/contact time with healthcare professionals, or additional situations not included in the original evaluation can be added by a minimum of 3 hours at the rate of $ 95 per hour paid in advance.
