We will guide you with the correct utilization of services that you or your loved one needs, such as Medicaid Institutional Care Program, Medicaid Waiver, Nursing Home Long Term Care Diversion, Diversion Transitional Programs, Optional Supplementation Systems and other services such as Veteran Benefits Resources and Medicare Services. It will be our goal to assist you and your loved one in coping with physical, emotional, psychological or financial situations. We consider all services, from Homecare, Alternative Housing, Clinical Social Services, legal services, insurances and any other healthcare services needed for a smooth transition.
